Dinner-Dance Journal Advertisement Order Form
A major part of our fundraising efforts from the Dinner Dance is through our Dinner-Dance Journal.
This year, we are publishing an 8 1/2” by 11” high quality journal which will include a four color cover.
The journal will be distributed at the Dinner Dance.

Prices are for black ink printing (except cover and Patriot pages.) Journal Advertisement, along with payment, must be received by April 21st, 2008. You may send your artwork by e-mail, in a PDF, Quark, Photoshop, MS Word or publisher file directly to MTM Printing Co. Inc. 23-21 College Point Boulevard, College Point, NY 11356.
